RISIKO KERUSAKAN INTEGRITAS KULIT PADA KLIEN STROKE DI RUANG AZZARA 1 RUMAH SAKIT ISLAM JEMURSARI SURABAYA

Abstract

ABSTRAK

            Kerusakan integritas kulit merupakan komplikasi dari penyakit akibat gangguan persyarafan, injuri tulang belakang dan klien dengan penurunan kesadaran sedangkan stroke merupakan 10 penyakit terbanyak di masyarakat yang disebabkan oleh beberapa faktor dan dapat terjadi secara berulang kali atau kekambuhan. Penelitian ini bertujuan untuk mengetahui risiko kerusakan integritas kulit pada lien stroke di Rumah Sakit Islam Jemursari Surabaya.

            Jenis penelitian ini adalah penelitian deskriptif dengan melakukan observasi terhadap klien. Populasi dalam penelitian ini adalah seluruh klien stroke yang di rawat di Ruang Azzara 1 Rumah Sakit Jemursari Surabaya dengan besar sampel 15 orang dengan menggunakan accidental sampling. Pengumpulan data dilakukan dengan observasi terhadap klien stroke. Variable bebas dalam penelitian ini adalah risiko kerusakan inegritas kulit variable terikat adalah klien stroke.

            Hasil penelitian ini menunjukkan bahwa terdapat faktor risiko yang dapat menyebabkan kerusakan integritas kulit diantaranya usia, persepsi sensori, status nutrisi, kelembaban, tekanan, mobilitas, gesekan, dan kebersihan tempat tidur. Selain untuk mengetahui risiko yang dapat menyebabkan kerusakan integritas kulit penelitian ini bertujuan untuk mengetahui adanya derajat kerusakan integritas kulit klien stroke, jenis stroke dan jenis kelumpuhan yang menyertai klien stroke yang memiliki risiko tinggi kerusakan integritas kulit.

            Perawat atau terapis diharapkan perlu adanya perhatian khusus bagi klien stroke yang menjalani perawatan untuk mengikuti progam terapi yang ada di Rumah Sakit. Serta perlu adanya perhatian khusus bidang keperawatan agar selalu melakukan pengkajian risiko kerusakan integritas kulit pada saat awal klien masuk di pelayanan kesehatan, khususnya bagi kelompok yang memiliki risiko  tinggi kerusakan integritas kulit.

ABSTRACT

RISK OF IMPAIRED SKIN INTEGRITY IN STROKE CLIENTS IN AZZARA ROOM 1 ISLAMIC HOSPITAL JEMURSARI SURABAYA

   Impaired skin integrity is a complication of diseases caused by neurological disorders, spinal injury and clients with decreased consciousness whereas stroke is the 10 most common disease in society caused by several factors and can occur repeatedly or recurrence. This study aims to determine the risk of impared to the skin integrity of on the lien stroke at the Islamic Hospital Jemursari Surabaya. This research type was descriptive research by doing observation to client. The population in this study were all stroke clients who were treated in Room Azzara 1 Hospital Jemursari Surabaya with a large sample of 15 people by using accidental sampling. Data collection was done by observation of stroke clients. Free variable in this research is the risk of damage inegritas skin of bound variable is client of stroke. The results of this study indicate that there were risk factors that can cause impaired to skin integrity such as age, sensory perception, nutritional status, humidity, pressure, mobility, friction, and cleanliness of the bed. In addition to knowing the risks that can cause impaired to skin integrity, this study aims to determine the degree of impaired to the skin integrity of the client's stroke, the type of stroke and the type of paralysis that accompanies the client stroke that has a high risk of impaired to skin integrity. Nurses and terapist should be special attention for stroke clients who undergo treatment to follow the existing therapy program in the Hospital. As well as the need for special attention in the field of nursing to always do risk assessment ofimpaired  skin integrity at the beginning of the client entry in health services, especially for groups who have a high risk of impaired to skin integrity
